  • I am an "out of network" provider. This means, I am not currently an outpatient provider for any managed care organization or insurance company. By not accepting insurance, I am able to provide a higher level of care that many patients appreciate.  This allows me to see patients for longer & more frequent sessions with better availability & significantly less wait times. At your visit, you will be provided with the necessary documentation if you wish to seek reimbursement from your insurance company. Full payment is due at each visit, and I will provide you with a receipt. You can submit this receipt to your insurance company for reimbursement directly to you at "out of network" rates.  Many plans offer "out of network" benefits, though some do not.  If you are planning to seek reimbursement, I encourage you to contact your insurance carrier to find out about your "out of network" benefit so that you can anticipate your out-of-pocket expense prior to scheduling an appointment.

    You may want to ask your insurance company the following questions regarding out-of-network reimbursement: 

    • Do I have mental health benefits?

    • Does my plan offer out-of-network benefits that will reimburse me if I am treated by an out-of-network provider?  If so, what percentage of the cost will I be reimbursed? Is my reimbursement based on a maximum allowable charge determined by the insurance company?

    • How long will it take for me to receive reimbursement?

    • Is my benefit limited to “parity” diagnoses (certain psychiatric conditions which are covered the same as medical diagnoses)?

    • What is my deductible and has it been met?

  • Payment for services is due in full at each visit.  I accept cash, checks, debit cards, and credit cards.  I ask that a credit card be kept on file for future payments and missed appointments.  

  • All information is strictly confidential with the following exceptions:

    When you give me written permission to release the information to someone

    When legal and ethical responsibilities require me to disclose information for safety reasons, such instances of a client threatening harm to self or others

    When ordered or subpoenaed by the court

    By law, I must report abuse of children or mentally disabled/vulnerable adults to Child or Adult Protective Services

  • I require 48 hours cancellation notice for any appointment which you are unable to keep. Full fees are charged for missed appointments and cancellations with less than 48 hours notice.  Please note that insurance does not reimburse any portion of these missed appointment fees.

  • I typically write prescriptions for refills during our visits.  If you need a refill between visits, please call 720-295-2456 to request a refill. Please allow 48 hours for these refill requests.

  • I provide care in Boulder, CO both in person and virtual care appointments. I use Zoom for telemedicine.  You will receive a zoom link 24hours prior to your meeting. You can use your cellphone or computer as long as you have a good internet connection and a camera and microphone.  

    When you connect to zoom, I will receive an alert that you are in the virtual “waiting room.” And I will start the visit with you at that time.

    Please call the office 720-295-2456 or text the office phone number if something is not working correctly.  If you text, please include your name so that I know who sent the text.
